- Income Certification
- Petition for Extension of Financial Aid 1 Use this appeal form if you have been disqualified because of not completing 67% of your attempted units or maintaining a 2.0 GPA. It may be submitted with a certification from a counselor that you have met and discussed your progress issues or a quicker solution would be to attend a group counseling session for students who have been disqualified. You may call the Counseling Office at 310-233-4229 or Financial Aid Office at 310-233-4320 to register for a session. There should be group sessions offered in mid-June and in August 2013.
- Petition for Extension of Financial Aid 2 Use this appeal form if you have an Associates Degree or more than 90 attempted units. It must be submitted with an educational plan from a counselor.
- Confirmation of high school graduation is required for Entitlement Cal Grant recipients. Please complete and submit this form if it is requested. Directions are included.
