EOPS Progress Report
EOPS progress report is due during the 2nd contact to an EOPS counselor. This is an requirement for EOPS students.

EOPS Workshop Verification Form
Workshop form should be signed by the workshop facilitator. EOPS students are required to attend one campus workshop per semester.​

​EOPS Mutual Responsibility Contract
The contract students agree to during orientation that outlines the students' responsibilities and requirements to maintain good standing with the program and eligibility for all program services and benefits.
