Students are required to ensure the following:

Have applied to LAHC and have a Student ID number
Please log into the form using your LAHC account
Complete the form entirely
Attach the appropriate transcript or AP score report if you have not submitted official ones to Admissions & Records

Unofficial copies of transcripts are accepted to clear Pre-Requisites

Students will be notified through their LACCD email address once the pre-requisite has been cleared or if additional information is required.
